Operasjon - Perioperativ hypotermi, forebygging hos voksne

Hensikt

Forebygging og håndtering av utilsiktet hypotermi for å hindre postoperative komplikasjoner hos voksne operasjonspasienter. Hypotermi gir økt risiko for infeksjon, blødning, kardiovaskulære komplikasjoner, trykksår, forlenget sykehusopphold og død.

Målgruppe

Avdelingsleder/seksjonsleder har ansvar for at prosedyren er kjent, tilgjengelig og blir fulgt.

Den enkelte helsearbeider som har ansvar for voksne operasjonspasienter med risiko for utvikling av hypotermi har et selvstendig ansvar for å følge prosedyren, dvs.:

Preoperativ fase på sengepost

Som en del av de preoperative forberedelsene må pasientansvarlig sykepleier/hjelpepleier i samarbeid med kirurg og anestesipersonell vurdere risiko for perioperativ hypotermi og evt. konsekvenser av dette før pasienten overføres til operasjonsavdelingen (1, 8).

Ved tilstedeværelse av minst to av følgende forhold regnes risikoen som høy

1 time før innledning av anestesi

Hos alle pasienter med kjernetemperatur under 36,0 ºC:

Intraoperativ fase

Trykkavlastende varmemadrass kan være et alternativ til aktivt varmluftsteppe, spesielt ved store operasjonsfelt der det er vanskelig å få god dekning av hud med aktivt varmluftsteppe (4, 22, 23, 25, 31, 32).

Postoperativ fase

Pasienten bør ikke overføres til sengepost med mindre temperaturen er minst 36,0 ºC (1, 8).

Hold pasienten behagelig varm når vedkommende er tilbake på sengepost

Hvis pasientens kjernetemperatur faller til under 36,0 ºC på sengepost:
